Prospective participants must:
- Be 18 years of age or older with one or more chronic medical condition(s) that require ongoing monitoring and supervision.
- Have impairments that limit the individual’s ability to perform everyday activities of daily living.
- Have a Health Assessment completed by their primary care physician.
